What exactly is a helix piercing?
The helix piercing is located on the outer, upper edge of the auricle. This area consists not of soft tissue (like the classic earlobe) but of firm cartilage (cartilaginous tissue).
The great thing about the helix is its extreme versatility. The upper ear rim offers plenty of space, which makes it wonderful for combining several piercings:
- Double or Triple Helix: Two or three piercings placed directly one below the other create a particularly harmonious look.
- Forward Helix: Located on the opposite side, on the front cartilage fold directly at the base of the head.
The Process: Why Ella Never Uses a Piercing Gun
When you come to Ella’s studio, hygiene is the absolute top priority. Your ear will be disinfected, and Ella will precisely mark the spot with a sterile pen. You always have the final say – only when you are 100% satisfied with the placement will we proceed.
One thing is an absolute rule at Old Dragon Ink: We never use an ear piercing gun! Piercing guns are designed for soft earlobes (and even then, they are controversial). If used on cartilage tissue, the tissue is literally shattered (micro-tears occur), which can lead to massive inflammation and unsightly cartilage overgrowth (keloids).
Ella performs the helix piercing with a sterile, razor-sharp hollow needle (venous cannula). This is extremely fast, maximally tissue-friendly, and creates a clean channel for the jewelry.
The Pain Factor: A Quick Prick, a Quiet Crunch
The question of all questions: Does it hurt a lot? Since pain perception varies from person to person, there is no general answer.
Fact: Cartilage tissue has less blood flow and fewer nerve endings than other body parts. The actual piercing is usually just a brief, dull pressure or prick that passes in a few seconds. What surprises many clients with a helix piercing is the quiet “crunch” they can hear as the needle penetrates the cartilage, as the piercing is so close to the ear canal. But don’t worry: Ella has an incredibly steady hand and will guide you through those seconds completely relaxed with the right breathing technique.
Initial Jewelry: Why You Must Start with a Stud
Many clients come to us wanting to wear a tight, delicate ring (hoop) in their helix on the very first day. Ella often has to play the professional spoilsport here: A fresh helix is always pierced with a titanium stud (labret stud).
Why is this so important?
- Swelling: The ear will inevitably swell after piercing. The initial stud is chosen to be slightly longer to allow the tissue this space. A ring would immediately exert pressure and cut into the flesh.
- Movement: A ring moves with every turn of the head and constantly pulls bacteria and crusts into the fresh piercing channel. A straight stud lies still in the tissue and heals much faster.
After about 6 to 8 weeks, you’ll come back for your downsize appointment. Once the swelling has subsided, Ella will swap the long bar for a perfectly fitting, shorter stud or (if healing allows) your desired ring.
Healing and Aftercare: Ella’s Golden Rules for Your Helix
Here’s the unvarnished truth: a helix piercing is a diva. Since cartilage is not supplied by its own blood vessels, healing takes significantly longer than with an earlobe piercing. You should expect 3 to 6 months (sometimes even up to a year) for the helix to heal completely and smoothly.

- Hands off! Never touch your fresh piercing with unwashed hands.
- Don’t twist it! A persistent myth says you have to twist piercings so they don’t get stuck. Titanium does not get stuck! Every twist reopens the delicate healing channel.
- Use aftercare spray: Gently clean the piercing twice a day with sterile saline spray or the aftercare product Ella recommends (e.g., Prontolind or Octenisept).
- The travel pillow trick: The biggest enemy of the helix is pressure during sleep. If you lie on the piercing, it will heal crookedly or become inflamed. Ella’s secret tip: Get a travel neck pillow and place your ear exactly in the hole in the middle when you sleep. This way, your ear floats pressure-free in the air!
- Caution at the hairdresser: Tell your hairdresser beforehand. Combs and towels that get caught on the fresh stud can hurt like hell.
